I consent to the treatment of any minor injuries of my child and release, hold harmless and indemnify 3DKids Rooted and the Church and their officers, directors, agents, employees, and volunteers from any and all liability, claims, and costs arising from or growing out of such treatment. I authorize the Church or any adult in whose care the minor has been entrusted to consent to any x-ray, examination, anesthetic, medical, surgical, or dental diagnosis or treatment and hospital care to be rendered to the minor under the general or special supervision and on the advise of any physician or dentist licensed under the provision of the Medical Practice Act on the medical staff of a licensed hospital, whether such diagnosis or treatment is rendered at the office of said physician or at said hospital. I agree to be liable and pay all costs and expenses incurred in the connection with such medical or dental service rendered to the child listed on this form.

I have read the below Drop Off & Pick Up Procedure *

Thank you for registering your child for Illumination Station VBS! Child drop off begins at 5:45PM. We ask that you not drop off your child/children before then, as we won’t have proper supervision available yet. When you and your child/children arrive at First Baptist Forsyth for VBS, your child will need to be checked in at the iPads located in the lobby. You will enter your phone number, which will bring up your child/children. Touch the “Check In” button, and the name tag labels will print. A parent pickup security label will also print. You will need to present this label (or a picture of it) at 8:30 parent pickup. When you arrive to pick up your child/children, please join the line in the lobby. The VBS Directors will be at the main entrance to the worship center and will speak your child/children’s name into microphones to call them to go home. There is no perfect process for pickup time, so we appreciate your patience!
